We went with this one: http://www.dbandart.com/ja/litho2.html
with a custom text. The most comprehensive source for texts we found was http://artketubah.com/Ketubah/ketubah_text_conservative.shtml

More internet recommendations:

We couldn't find any invitations we liked in our price range, so we bought a color ink-jet printer, this paper, http://italianartstore.bizland.com/store/Fabriano_Medioevalis.html and printed our own. Let me know if you want copies of the invitation and program files (all are in Word).

Also, I got a dress on ebay and had it altered. The seller claimed that the dress had never been altered or pressed, and the alteration shop confirmed it--apparently, once these disposable dresses have been pressed, the fabric melts slightly and they can't be altered again. The whole process worked surprisingly well (I knew what sort of dress I wanted, couldn't find it in the first couple of stores I looked in, and found it immediately on ebay). The whole thing took relatively little time and energy.

Best,
Eliz.
